Fee Payment Information

The final deadline for meeting registration and mailing of fees is Monday, March 27, 2002. After March 11th, registration will be allowed on a space-available basis only. The fee for registration is $150.00. The registration fee will cover the cost of the published proceedings (each attendee will get a copy).

Do not send checks after March 27th. If you miss the mail-in deadline, you may pay at the meeting (checks only, please). Please make every effort to mail your check in advance. This will streamline the check-in process upon your arrival at the meeting. Please make checks payable (in US dollars) to "Gillett Symposium" and mail your payment to:

How to Register

There are three ways to register for the Fred Gillett Symposium: World Wide Web, US Mail or FAX. Please do not submit more than one registration form.

1. Registration by Web Form: You may register using the online World Wide Web registration form at the URL:

Please follow the instructions for filling out the electronic form very carefully. Please print out a copy of the form for your confirmation. We recommend that you bring a copy of your registration with you to the symposium in the event of an error in processing your registration.

2. Paper form (MAIL or FAX): To submit a paper form by mail or fax, please print the form by using either the Word Document or PDF version below. Before you submit your registration by mail or fax, make sure to complete all the applicable areas. You may MAIL or FAX this form to the address and fax number provided above.



If there are additions or corrections to be made to your registration, do not submit another registration form. Please contact for fax details to 520-318-8360. Requests by phone will not be accepted.
