WIYN 0.9m Observing Run Preparation Form

Fill out this form and email to kpno@noao.edu six weeks before the start of
each scheduled observing run. This information is required to coordinate
support staff and resources. Failure to submit your ORP on time may 
result in forfeiture of requested filters, delays in starting up, or 
loss of lodging accomodations! Any questions should be directed to
heidis@noao.edu. Note: You will need to provide your own transportation
to/from Kitt Peak.
